The Christiana Care Health Care System this week earned the Eisenberg Patient Safety and Quality Award for its technology-based Care Link program.

The award is given by the Joint Commission and the National Quality Forum, two prestigious performance trackers of the health care industry.

The Care Link program allows patients and providers to access an information network spanning the local health care system, tracking everything from lab results and pharmaceutical use to visits to a specialist or the emergency room.

The goal of the program is to improve outcomes for the individual as well as better understand public health concerns through predictive analysis of health data.

“The missing link to optimal health is addressing the gaps between sicknesses and health crises. It is ensuring that patients’ social and behavioral health needs — with their great impact on health — are being met, in addition to their medical needs,” said Sharon L. Anderson, RN, BSN, MS, FACHE, chief population health officer, senior vice president of Quality and Patient Safety, and president of Care Link.

“Care Link’s unprecedented predictive analytics capabilities and innovative work in care coordination improves health care safety, quality, value and access to care for all throughout our state and region,” she added.
