Bayhealth partners with Unite Us Delaware

Bayhealth has announced its partnership with Unite Us Delaware, a technology company that coordinates health and social care services. The partnership will help Bayhealth provide care beyond the hospital by identifying social determinants of health and ensuring patients have every tool they need at home to stay healthy.

Bayhealth has been screening for social determinants of health since March 2020 on the inpatient side and primary care side. Clinicians assess patients to identify possible vulnerabilities, including access to food and other resources. If a patient lacks basic health necessities, they will be referred to Unite Us Delaware and connected to organizations that help address any inequities.

Together, Bayhealth and Unite Us Delaware will be able to securely connect people in need with community-based resources that can be difficult to navigate. The network provides a central point of contact where healthcare providers, social service organizations, and individuals can securely access and refer people to needed services while monitoring progress and measuring outcomes.

A patient’s care team will receive real-time updates throughout the process. Providing at-risk patients with access to food, clothing, shelter or other assistance increases their chance at staying healthy after receiving medical care.

“Unite Us Delaware enables the Bayhealth care team to refer patients with identified social needs to available community resources and follow up on their engagement with these resources,” Bayhealth Clinical Integration Director Evan Polansky, JD, MBA, said. “This is a vital piece of Bayhealth’s Social Determinants of Health program.”

The partnership will allow Bayhealth to identify health disparities and proactively address barriers patients face, directly supporting Bayhealth’s mission of strengthening the health of our community.
